This page contains forms that can be copied and used to keep records current. In the event you do not have enough Scannable forms to post the meals/attendance daily you will be able to print out the Daily Meal Worksheet and Menu Plan to keep your records current until scannable forms are available.
If you claim on line and your computer breaks down, having forms printed out in advance will allow you to keep your records current.
Additonal forms are provided to qualify parents/children for meals that you claim for the children in your care, etc.
Documents are sorted by type for easy reference. Not all forms are available in Spanish but we welcome your call and will provide assistance to you in Spanish for completing the forms
